InfoStar Nomination Form

Nominations for InfoStars may be submitted by the nominee's colleagues. InfoStars will be selected by a Dialog review committee, based on the criteria in the "On Being an InfoStar" document. To submit a nomination, please complete and send this form to the Quantum2 program managers at . One set of InfoStars will be announced each year at the ALA Midwinter Meeting in January, SLA Annual Conference in June and another at the London Online Information show in December.
